Role Overview

Exeter Learning Disability Partnership seeks a Registered Nurse (RNLD, RMN, RN) to work as a Community Nurse with The Northeast Essex Community Learning Disability Team, based at Lexden Hospital, Colchester. The role focuses on person‑centred care for adults with learning disabilities living in the community, all environments, and a commitment to integration with mainstream services in line with national and local policy.

Responsibilities
  • Assess the health care needs of people with learning disabilities across a range of environments and implement interventions as required.
  • Work in an interdisciplinary way to assess service users’ needs, devise care plans and evaluate outcomes.
  • Collaborate directly with service users, carers, colleagues and other service providers.
  • Lead health promotion activities for people with learning disabilities, ensuring inclusion into mainstream services.
  • Deliver training and education to other professionals, general practitioners, carers and families.
Qualifications & Competencies
  • Professional registration with the Nursing and Midwifery Council (NMC) or equivalent.
  • Compassion, resilience and a dedication to empower and support service‑user independence.
  • Effective communication skills across a variety of methods with service users, MDT members, carers/family and agencies.
  • Strong record‑keeping practices and a commitment to maintaining high standards.
  • Good understanding of legislation and national guidance relating to the care and support of people with learning disabilities.
