A community healthcare provider in England is seeking a Senior Community Nurse to join their Urgent Community Response Service. The role involves providing face-to-face care in patients' homes, advocating for patient choice, and working collaboratively with a multidisciplinary team. The ideal candidate should have strong community nursing experience and excellent communication skills. The position requires some weekend work with service hours from 8 am to 8 pm, 365 days a year.

This is an exciting opportunity to be part of our Urgent Community Response Service in North Norfolk as a Senior Community Nurse. The role will predominantly involve face‑to‑face nursing care and assessment within a patient’s usual place of residence, with an element of triage for the Urgent Community Response Team on a rota basis as well as other supportive activities as part of the multidisciplinary team (MDT). It is a 7‑day a week service with some weekend working expected. Hours of service delivery are 8 am‑8 pm, 365 days per year.
