A leading health organization in Wales is looking for a skilled Mental Health Nurse to coordinate care for clients with severe mental health problems. This role involves developing treatment plans and conducting risk assessments within a supportive multi-disciplinary team. Candidates with the ability to speak Welsh are encouraged to apply. This position is crucial for providing effective support to vulnerable individuals.
To be responsible for the care co-ordination of a designated caseload of clients with severe and enduring mental health problems. To ensure that the Care and Treatment Plans are adhered to and that standards are met in line with the Mental Health Measure (Wales) 2010. To act as a point of contact for GP's and other interface services to offer advice, support and signposting to appropriate agencies. Formulate and develop standard and complex outcome focused care and treatment plans. Ensure that a comprehensive risk assessment is completed and documented for actual and potential risk.
