A community healthcare provider in Havant is seeking a motivated nursing professional to join their Integrated Care Team. The role involves delivering services to a GP Practice population, including home visits and supporting long-term patients with mental health issues. Applicants should have relevant healthcare experience, strong clinical abilities, and excellent communication skills. A full driving license is essential for applicants. To work within the multi-disciplinary Integrated Care Team that delivers community based services to a GP Practice population, undertaking nursing tasks within agreed parameters, set out by the registered practitioners.
The job will include home visiting and lone working without direct supervision.
The scope of the role will focus on supporting admission avoidance, early supported discharge and promotion of independence and assisting in the effective management of patients with long term conditions and older people with mental health problems.
