Job description
Overview

We step in to support people who are discharged from acute hospitals and those who need an urgent community response. The role requires short‑term/interim support in their homes. The team is committed to caring and supporting people at home and working over a seven‑day rota.

Hours

Various shift patterns including unsocial hours, weekends and bank holidays.

Location

Base: Florence Nightingale Community Hospital. Area Covered: Derby City and surrounding area.

Travel requirements: You must be able to meet the travel requirements of going to patients’ homes. This will require a UK driving licence and access to a vehicle. DCHS has a mileage policy and car lease scheme available to support staff.

Responsibilities
  • The successful candidate is expected to work and document to a high standard and work in a way that reflects the DCHS values.
  • To maintain standards, ensuring quality of patient care through the development of a range of knowledge and competencies to support clinicians.
  • To assist all clinicians in delivering agreed programmes of treatment and care where appropriate and to work as an effective member of the wider multi‑disciplinary team.
  • To carry out individual care without direct supervision in the community.
  • To provide support to patients to optimise their health and independence.
  • Be willing to support Perth House in Chaddesden when and if staffing cover requires.
