Job description
About the Role

As a Community Care Worker, you will work closely with Occupational Therapists and other professionals to deliver effective, person-centred support. You’ll be involved in:

  • Carrying out assessments to identify and address daily living needs
  • Supporting adults through equipment solutions, minor and major adaptations
  • Working in partnership with housing providers, health professionals, and other agencies
  • Promoting independence and wellbeing through a strengths-based approach
  • Ensuring accurate and timely case recording (Mosaic experience desirable)
  • Safeguarding adults and embedding safe practice in everyday work

You’ll be supported by an experienced management team, senior practitioners, and a wider network of Social Care Practitioners. We pride ourselves on providing strong supervision, guidance, and a welcoming team culture.

What You’ll Bring
  • Knowledge and/or experience of working in a person-centred, holistic, strengths-based way
  • Experience of assessing activities of daily living
  • Understanding of physical disabilities, learning disabilities, and/or mental health needs and their impact on daily living
  • Experience of equipment and adaptation solutions
  • Ability to manage and prioritise a caseload effectively
  • Strong communication skills to build positive relationships with clients, carers, and professional partners
  • Good IT, organisational, and time management skills
  • Understanding of relevant statutory legislation in adult social care and health
  • Knowledge of safeguarding principles and commitment to safe practice
  • Personal resilience and a positive approach to risk-taking
  • Ability to travel across the county for home visits

Essential qualifications:

  • NVQ Level 3 in Care (or equivalent qualification/experience)
  • Valid UK driving licence and ability to travel for work
  • Ability to communicate effectively in fluent English
