Overview
Role: Community Care Coordinator
Mind in Haringey is an independent charity providing mental health services in Haringey since 1989. We have a broad range of services and projects, and we are seeking a Community Care Coordinator to support our service users alongside statutory partners to increase wellbeing, confidence, social integration, and independence. The successful candidate will work with service users during periods of crisis and support their recovery journey using a person-centred approach that places the views of service users at the centre of planned support.
The role involves working in the community and with the wider team, including our partner organisation North Middlesex NHS Foundation Trust Emergency Department, to keep the community informed about our services and to maintain Mind in Haringey’s strong reputation as a leading mental health provider in the borough. This position offers the opportunity to see the positive difference our services make in local residents’ lives. The staff are a lively, committed group who work well as a team.
Responsibilities
- Support individuals through the referral procedure, on their initial visits, during assessment, and throughout their time with the service.
- Devise and implement care/wellbeing plans for each service user.
- Signpost service users to internal and external services to ensure access to the most beneficial support, including in-house courses such as Improving Financial Resilience, Employability/Volunteering Skills, Coping with Life Skills (CBT psychoeducational group work), or Cooking on a Budget.
- Support individual recovery using tools such as the Outcome Star, Wellness and Recovery Action Plans.
- Provide support and advice to service users regarding Universal Credit and Personal Independent Payments, and assist with filling out personal forms.
- Work with project staff to monitor outputs and outcomes.
- Identify and research venues/locations for Mind in Haringey to advertise its services.
- Encourage user participation in planning, development, and evaluation of mental health services and support the work of the Service User Forum.
- Collaborate with other community mental health services to develop and maintain an effective, high-quality service.
- Build positive relationships with voluntary and statutory sector partners in the borough to enhance cross-sector referrals.
- Support mental health promotion within the local community.
- Maintain service user records and collect data as required by funders to support analysis, audit, and evaluation; produce written reports as required.
- Input data onto the service user database.
- Participate in training relevant to the delivery of the service, regular clinical, line management supervision, and annual appraisal.
- Maintain standards of care regarding health and safety, equality, confidentiality, and individual/personal choice; work within Mind in Haringey policies and procedures, with specific reference to Safeguarding of Vulnerable Adults and Child Protection policies.
- Undertake other appropriate duties relevant to the post as directed by the Operations Service Manager.
Qualifications and Experience
- Experience: 2/3 years in mental health settings or health and social care with vulnerable adults.
- Demonstrable track record of supporting vulnerable people with mental health problems.
- Knowledge of the benefits system including Universal Credit and Personal Independent Payments.
- Knowledge of health and social care terminology and pathways; awareness of health and social care legislation.
- Awareness of local community resources; safeguarding knowledge; awareness of the Mental Capacity Act; data protection and confidentiality.
- Awareness of equal opportunities legislation; knowledge of the assessment process.
- Experience of providing advice and support (including filling out personal forms) and/or coaching; ability to write in coherent English; ability to work with a range of people and be a strong team player.
- Ability to work with minimal supervision, plan workloads, and organise varied projects and activities; computer literacy in software packages.
- Good organisational and communication skills, including presenting oral and written reports; strong negotiation skills; positive attitude; commitment to equal opportunities and teamwork.
- Experience of project planning and working in a not-for-profit environment is desirable.
Salary and Conditions
- Salary: pro rata £27,300 per annum
- Hours: 20 hours per week
- Contract: Fixed term until 31 March 2026 (with possible extension)
- Annual Leave: pro rata of 28 days per annum (including bank holidays)
- Responsible to: Projects Manager; Accountable to: Chief Executive Officer
- Based at: Mind in Haringey, North Middlesex Hospital
