Qualifications

• Registered with Social Work England.
• Previous experience within adult services/community teams is highly desirable.
• Strong knowledge of relevant legislation, policy, and practice guidance.

Responsibilities

• Undertake assessments and reviews in line with Care Act requirements.
• Support adults with complex needs to achieve positive outcomes.
• Work collaboratively with multi-disciplinary teams and partner agencies.
• Contribute to safeguarding practice and risk management.
• Promote independence and well-being across the community.
