Location: Northwest England (within a 100-mile radius)

Amore Groupis seeking a passionateClinical Transitional Leadto support new care packages and ensure compliance with Amore’s policies and CQC regulations. This dynamic role involves working across the Northwest to assist with service user transitions, supporting high-quality care delivery.

About the Role

As aClinical Transitional Lead, you will:

· Ensure care packages meet Amore Group's high standards and align withCQC regulations.

· Collaborate withmobilisation managersand staff teams to deliver high-quality care.

· Cover a100-mile radius in the Northwest, supporting service user transitions.

· Receive training inclinical care, behaviour management, andperson-centred care.

Key Responsibilities

· AssistMobilisation Managersin leading staff teams for new care packages.

· Be thelead contactfor sub-teams supporting new service users.

· Plan rotas and ensure emergency cover is provided as needed.

· Ensure care aligns with theCare Standards Act 2008and relevant legislation.

· Help createperson-centred care plansthat reflect the needs and aspirations of service users.

· Facilitate community engagement opportunities for service users, including work, education, and recreational activities.

Qualifications & Skills

We are looking for candidates who:

· Hold aDiploma in Health and Social Care Level 3or equivalent.

· Have completedMandatory Trainingand additional training for complex care service users.

· Arecommitted, caring, and enthusiasticabout delivering excellent care.

· Hold afull clean driving license.

· Understand theCare Standards Act 2008and other relevant legislation.
