We are looking for a highly skilled, enthusiastic, dynamic self‑motivated leader to lead our Frailty Team based at Clarendon, Hebburn. Would you enjoy the challenge of providing senior support and clinical leadership in the delivery of high‑quality care for patients in the community setting? If so, this could be the job for you.
You will be responsible for the leadership and management of the Frailty Team consisting of Band 7 specialist nurses. You will be the post holder accountable for the overall management and development of the team and for day‑to‑day governance and assurance.
The Frailty Team normally works Monday to Friday 08:30‑17:00. The Trust adheres to a flexible working policy.
The post holder will be accountable for the overall management and development of the team, supervision of staff, ensuring coordination of services, optimising the cost‑effective use of resources, line management of the team and overall responsibility for delivery of patient/client care.
To lead, motivate and support the team in delivering innovative models of care, continuously improving service, ensuring systems are in place to support clinical governance, quality monitoring, ongoing research and audit.

Accountable for overall management and development of the team/service, deployment and supervision of staff, coordination of services and optimisation of resource use.
Line management of the team, overall accountability and responsibility for delivery of patient/client care through effective clinical and managerial leadership.
Lead, motivate and support the team approach to delivering new and innovative models of care for patients and clients.
Lead a continuously improving service by ensuring systems and processes to support clinical governance, quality monitoring and ongoing research and audit of practice.
Ensure systems and processes maintain patient safety and facilitate development of clinical expertise and advanced practice within the team.
Work autonomously, assessing patient/client needs, initiating investigations, determining lines of care, and initiating appropriate holistic, research‑based health information and care.
Act as a resource for all members of the multi‑disciplinary team, providing expert advice concerning patient/client management.
Responsible for ensuring professional standards are reflected in practice and overall monitoring the standard of care.
Improve the patient/client journey by increasing access to assessment and appropriate care and treatment.
Work collaboratively with multi‑disciplinary teams to ensure practice is efficient, effective, evidence‑based and safe.
