TheLTHT Operations team are seeking an experienced and motivated senior nurse,with an acute nursing background, to join our team of Clinical Site Managers.The team work across all Leeds Teaching Hospitals Trust and cover 24 hours,365 days per year. We are seeking anindividual with significant senior nurse experience, working at a minimum ofa Band 7 level.
Thisexciting and high profile post demands a motivated and responsive leader witha focus on both the provision of high quality care and improving performance.The successful candidate will need to be able to work well under pressure andexhibit strong leadership and communication skills with a clear understandingof organisational and national service requirements. Thejobholder will be responsible out of hours for:
themanagement of the emergency and elective admission process across the Trust,safe and appropriate patient placement and the achievement of related localand national Trust standards.
theclinical management of the site providing leadership and support to all wardsand departments.
theoperational management of the Trust, implementing policies and procedures asrequired and providing support and advice to Trust employees.
Thejobholder will participate in and co-ordinate the Trusts emergency responseteams to Fire, Security Alerts and other clinical emergencies. They will takea lead role in the co-ordination and first line management of Major andSerious Incidents occurring out of hours as required. They will be linemanaged by the clinical site manager team leader and out of hours isresponsible for reporting to the on call manager according to establishedescalation procedures. The jobholder is responsible for ensuring the timelyand appropriate placement of patients requiring admission to the Trusts bedbase and for responding to the escalation procedure from the EmergencyDepartments and will have responsibility for the management of patient flowsTrust wide. they will provide leadership to teams both in and out of hoursand will co-ordinate the hospital handover from Clinical Service units to theCorporate Operations Team focusing on Patient flow, Safety and Performance.

PRINCIPAL DUTIES & AREAS OFRESPONSIBILITY
Monitor and maintain patient flowacross the Trust sites.
On behalf of the Clinical ServiceUnits actively manage and be accountable for the efficient and effective useof the Trusts bed base out of hours.
Act on the behalf of the ClinicalService Units when making decisions about the admission ofunscheduled/elective patients.
Work with Clinical Directors,General Managers, Heads of Nursing and Matrons to ensure appropriate patientplacement is delivered within Trust frameworks and to support the delivery ofthe Trusts short to medium term objectives.
Ensure that operational policiessupport effective processes and are communicated, implemented and audited.
Working with Trust colleagues, takea lead role in maintaining patient flow using the standard operatingprocedure for managing capacity and demand.
To monitor the trigger factorsassociated with the Trusts Acute Care Escalation Plan.
Working with internal and externalcolleagues/agencies, to maintain an effective discharge process thatmaximises the use of community provision.
Review of service delivery toensure that delays in patient placement are identified and acted upon,ensuring ongoing improvement in patient access.
Maintain an overview of actual andpotential bed capacity and ongoing demand across the Trust, analyse andmitigate pressures.
To maintain an overview of timelypatient flow across the site with particular focus on admissions through theEmergency Department.
To determine the best use ofavailable capacity
To consider the elective admissionprocess reconciling the demand for optimal activity with the need to providesufficient capacity for urgent admissions.
The immediate co-ordination andfirst line management of the Trusts emergency response to Fire, Majorincidents, Security alerts and Serious Untoward incidents.
To be the on site managementresource for staff out of hours utilising LTHT policies and procedures.
To liaise with Security manager andPublic Relations and, on behalf of the Trust, external agencies such as thepolice and media.
To re deploy staff in order tocover for unexpected sickness or absence.
To provide support and advice toall Trust employees.
Liaise with managers regarding theclinical and operational management of their areas.
To ensure that critical/untowardincidents and safeguarding issues are reported as per locally agreed TrustPolicies.
Valuefor Money
Out of hours be responsible for theallocation of staff to all areas, ensuring adequate staffing cover isprovided on a daily basis. Review nursing/support staff establishments toensure skill mix reflects current workload requirements and represents bestuse of resources.
Assess the impact of extra hoursworked by staff on their performance and the needs of the service.
Qualityof Care/Clinical Governance
Work with members of the ClinicalService Units to deliver high quality, timely health care. Contribute to theimplementation of National Service Frameworks (where applicable) and NationalPolicy.
Work closely with Clinical ServiceUnits to secure and ensure the highest standard of clinical care is providedwithin available resources.
Ensure all processes encompassmeasures to ensure smooth transition of patients through the care pathway,for example, timely admission and discharge processes.
Ensure that clinical governanceprocesses encompass measures to ensure the quality of the fundamental andessential aspects of care.
Ensure nurses work within the TrustNursing Policy and Practice Guideline framework.
Communicate effectively throughdaily contact with all disciplines in the relevant site and associated sites.Attend the Daily Operational Performance meetings and chair the patient flowmeetings.
Ensure clinical and non-clinicalincidents are reported in accordance with Trust policy and agree actions inresponse to these.
Maintain a safe workingenvironment, being aware of the requirements to comply with Health and Safetyregulations. Ensure equipment is maintained according to manufacturersspecifications and necessary repairs are undertaken. Ensure staff are trainedand supervised where appropriate in the correct use of equipment.
Address the concerns of patientsand service users and respond to their suggestions for local qualityimprovements. Lead the investigation and management of complaints out ofhours through local resolution and in accordance with the Trust complaintsprocedure, liaising with the Patient Relations department where necessary.
Support the implementation of aPatient Advice and Liaison Service (PALS), being a focal point for advice topatients and carers and local resolution of complaints.
Work with Heads of Nursing andMatrons to ensure the implementation & delivery of the Nursing Strategy.
Act in accordance with the Nursing& Midwifery Code of Conduct.
Educationand Training
Undertake relevant staff appraisalsand ensure staff have personal development plans in accordance with Trustpolicy and support other senior staff in this process.
Promote evidence-based practicewithin the patient admission/discharge process. Support staff in undertakingresearch/project work.
Leadership
Act as a positive clinical rolemodel through the provision of support and advice to staff. Lead by examplethrough visible leadership to motivate and empower others.
Ensure that appropriatearrangements are in place to provide leadership, support and motivation tohelp immediate subordinates and others to understand and achieve the requiredresults.
Ensure effective workingrelationships and communication is maintained between all grades of staff andprofessional groups within the designated area, working within amultidisciplinary team.
Act as a positive role modelthrough the provision of support and advice to staff. Lead by example throughvisible leadership to motivate and empower others.
Contribute to the management ofsickness absence and staff capability in accordance with Trust policy.
