A healthcare practice in England is seeking a part-time Clinical Pharmacist to join their team. The role involves providing medication management and improving patient care outcomes. We are looking to recruit a Clinical Pharmacist to support our GP team at Abbey House Medical Practice.
The post holder is a Clinical Pharmacist who acts within their professional boundaries, working alongside a team of pharmacists in general practice within the wider PCN. They will improve patients health outcomes and the efficiency of the primary care team by providing direct, accessible and timely medicines and expertise including face to face contact with patients. The post holder will be an integral part of the general practice team, as well as part of a wider pharmacist network and multi-disciplinary team. They will work to optimise medication issues to improve patient care and safety and support clinical staff in the management of patients.
Clinical pharmacists have a key role in supporting delivery of the new Network Contract DES Service specifications. For the new Structured Medications Review and Optimisation requirements this will include tackling over-medication of patients, including inappropriate use of antibiotics, withdrawing medicines no longer needed through NHS-led programmes such as low priority prescribing and medicines optimisation more widely. For Enhanced Health in Care Homes residents will benefit from regular clinical-pharmacy led medicines reviews.

This is a temporary position - Monday to Friday, part-time hours working 3 days a week.
