Are you a Clinical Pharmacist looking to make a real difference ina supportive, forward-thinking practice?

We have a highly skilled and well-led team of five Pharmacy Technicianswho work under the supervision of Dr Irma Murjikneli. Dr Murjikneli is anexperienced GP who has worked for many years on the Area Prescribing Committee.

Main duties of the job

We are looking for a full-time independent Prescribing Pharmacistwith experience of working in a clinical (ideally Primary Care) setting. Theideal candidate will be self-motivated, highly IT-literate, and have experiencein the leadership of a group of clinicians. Knowledge of change managementwould be advantageous.

The new role will lead the Technicians team through atransformation in which the Pharmacy Team takes responsibility for themanagement of Chronic Disease.

This role offers the chance to develop your clinical skills,contribute to innovative patient pathways, and work closely with our GPs, PharmacyTechnicians, Nurses, and the wider multidisciplinary team.

Job responsibilities
Primary Duties & Areas of Responsibility
Supervision, Developmentand Leadership of the Pharmacy Technician Team
RepeatPrescribing
  • Oversee and maintain the practice repeat prescribing policy,ensuring safe, efficient and evidence-based processes. Utilise and comply withresources such as Sussex Formulary and NICE CKS.
StructuredMedication Reviews (SMR)
  • Lead and continually improve the structured medication reviewprocess.
  • Undertake complex medication reviews (face-to-face, telephone orhome visit), focusing on frailty, polypharmacy, high-risk anddependence-forming medicines, and de-prescribing where appropriate.
MedicinesReconciliation and Continuity of Care
  • Provide clinical leadership for the medicines reconciliationprocess and supervise the pharmacy technician team in completing dischargesummaries, clinic letters and care-home admissions (new patient medicationrequests).
  • Identify and resolve medicine discrepancies, ensuring safetransfer of care and accurate prescribing following hospital discharge orattendance.
  • Ensure processes are in place to maintain continuity of medicinessupply for high-risk and vulnerable patients (e.g., care-home residents orthose using compliance aids such as blister packs).
MedicinesSupport and Clinical Advice
  • Act as point of escalation for medicine-related queries frompharmacy technicians, clinical staff, and patients.
  • Lead on MOIS, analysing prescribing data and implementing actionsto achieve quality and financial targets.
SafePrescribing & Governance (including MHRA alerts)
  • Lead on safe prescribing, including monitoring requirements forhigh-risk medicines and the de-prescribing of inappropriate ordependence-forming medicines.
  • Receive and implement MHRA alerts, product withdrawals andguidance from national and local sources.
  • Lead prescribing audits, present findings, and implementimprovements with the practice team. Work closely with the clinical andpharmacy technician teams to deliver required clinical audits.
ServiceDevelopment
  • Provide pharmaceutical expertise in the development andimplementation of new services, including clinical pathways and patientinformation resources.
InformationManagement
  • Analyse, interpret and present prescribing data to highlightrisks, trends and improvement opportunities.
  • Working closely with the Senior Pharmacy Technician in the Leadershipand Management of the Pharmacy Technician Team, for example appraisals, rotasand other HR functions
Chronic Disease Management
Long-TermCondition (LTC) Clinics
  • Lead and support LTC clinics aligned with QOF and Locally CommissionedService requirements. Priority will be given to the areas where further servicedevelopment is needed in the surgery.
  • Work collaboratively with GP Leads aswell as Specialist Nurses(e.g., Diabetes, Asthma, Hypertension) and develop the Pharmacy Technician rolewithin LTC management.
  • Work collaboratively with the wider practice team, communitypharmacies, PCN teams and neighbourhood practices to ensure safe and efficientpatient care.
  • Ensure prescribing follows theICS formulary and local shared-care/ NICE guidance.
  • Make recommendations to GPs regarding red / amber drugs and OTCprescribing.
  • Provide education and training to clinical and Pharmacy Technicianteams on medicines optimisation, prescribing guidance, and safety.
  • Support public health campaigns and provide expert advice relatingto medicines and public health programmes if needed.
  • Recognise the roles of other colleagues within the organisationand their role to patient care
  • Demonstrate use of appropriate communication to gain the co-operationof relevant stakeholders (including patients, senior and peer colleagues, andother professionals, other NHS/private organisations e.g. ICSS)
