Job description
Overview

The successful candidate will have experience in neurology – preferably in Multiple Sclerosis – and in case management. We are seeking a keen and enthusiastic person to provide high quality care to patients with MS across the community. You will possess advanced clinical assessment skills, as well as excellent communication and interpersonal skills. The ideal candidate would be passionate about caring for people with Multiple Sclerosis.

You will be covering the South Bank side of the Humber; the base can be discussed at interview. The post will require flexible working across the local area to meet service need, including clinics and home visits.

Responsibilities
  • Provide education, symptom management, medicine management, advice and support to patients in North Lincolnshire, Northeast Lincolnshire and the wider local area with a confirmed diagnosis of MS.
  • Deliver clinics across multiple locations in the North and Northeast of Lincolnshire and attend patients’ homes for home visits.
  • Work in partnership with a variety of services and settings to care for individuals with MS.
  • Actively collaborate with colleagues within the MS team to enhance the delivery of care and inform new practice.
  • Provide specialist resources and develop the knowledge of individuals with MS, their relatives and carers, members of the public, health professionals, statutory and voluntary bodies as appropriate.
  • Return specialist advice and guidance to staff across organisational boundaries, supporting teaching and education efforts.
Qualifications
  • Experience in neurology, preferably in Multiple Sclerosis, with case management experience.
  • Experience working with patients who have neurological conditions and evidence of ongoing professional development.
  • Advanced clinical assessment skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently.
  • UK driving licence and flexibility to work in different localities within the area.
