Job description

Responsibilities

• Provide high quality nursing care whilst working within the limitations of the role and understanding how to escalade when required.
• Work effectively as a member of the multidisciplinary team.
• Maintain and develop high standards of care.
• Work proactively to ensure that all patients’ expectations and needs are met in a timely manner.
• Respond to emergency situations with composure, maintaining patient safety and seeking immediate support whilst supporting the patient, family and other staff as required.
• Maintain clear, accurate and concise patient records in keeping with departmental and professional standards.
• Report near misses and untoward incidents, complaints, emergencies, injuries or medication errors via the trust reporting system and assist lead staff in investigating as required.

Professional Development, Education and Training

• Take on responsibility for personal development and support the development of others.
• Identify training needs in conjunction with line management.
• Be actively involved in and support the department’s education programme.
• Consolidate and learn new skills and competencies to provide holistic care.Undertake all mandatory training and specified training for your role as HCSW.

Communications and Working Relationships

• Communicate effectively with patients, carers and relatives as well as other members of the multidisciplinary team.
• Maintain patient dignity and confidentiality at all times.
• Maintain good professional working relationships with the team and the multidisciplinary team.
