A leading healthcare staffing agency is seeking a CHC Nurse Assessor Band 6 in the West Midlands area. This role involves conducting assessments and collaborating with families and professionals to ensure client care. The ideal candidate will be a registered nurse with NHS experience, skilled in managing complex cases. Competitive daily rate and ongoing contract offered.
CHC Nurse Assessor Band 6 Location: West Midlands area Rate: £300 per day Working Pattern: Monday to Friday, 9:00am - 5:00pm Team: All Age Continuing Healthcare (AACHC)
Conduct thorough assessments, reviews, and monitoring of care for clients funded via NHS Continuing healthcare and Funded Nursing Care
Act as Case Manager to facilitate, plan, and evaluate client care in collaboration with families and professionals
Maintain strong relationships with health, social care, and independent sector colleagues
Provide professional advice, training, and mentorship to colleagues where required
Participate in service development, clinical audits, and research to improve practice and patient outcomes
Ensure all assessments are completed in line with national frameworks, safeguarding, and statutory guidance
Registered Nurse with current NMC registration or equivalent
Educated to degree level or equivalent experience in a specialist area
Previous experience conducting Continuing Healthcare assessments for adults with complex needs
Strong knowledge of multi-disciplinary team working, safeguarding, and clinical governance
Excellent communication, negotiation, and conflict management skills
Ability to manage complex caseloads and work autonomously
Experience in providing advice and support across health, social care, and independent sectors
Master's in healthcare or related area
Experience in leadership roles and cross-boundary collaboration
Knowledge of commissioning arrangements, DOLS, and SEND agenda
Monday to Friday, 9:00am - 5:00pm
Contract: 3-6 months ongoing, ASAP - 4 weeks start
