A healthcare organization in the UK is seeking a proactive and dynamic case manager for their Acquired Brain Injury service. The Acquired Brain Injury (ABI) service is seeking a proactive and dynamic individual with strong communication skills to join the team as a case manager. The ideal candidate will have experience working with individuals with an acquired brain injury and a genuine passion for supporting their recovery and well-being. The role is suitable for an experienced band 6 practitioner looking to take the next step or someone already in a band 7 role who would like a new opportunity.
The post is based primarily within the Wirral team of the ABI Service which is an established and successful multidisciplinary community based team covering Wirral and Cheshire. The case manager role offers the opportunity for blended working, including face to face sessions, remote sessions via telephone and video and working from home. It will involve travelling around the Wirral.
We provide comprehensive support for individuals affected by acquired brain injury, including case management, vocational rehabilitation, and neuropsychological interventions. The service collaborates with universities to facilitate student placements, support quality improvement projects, and promote continuous learning opportunities.
The case manager will be an experienced therapist or healthcare professional who will proactively coordinate interdisciplinary collaboration between statutory and independent provider services to address the needs of clients with acquired brain injuries living on the Wirral. The postholder will take lead responsibility for the co‑ordination and delivery of initial screening, risk assessments and evidence‑based interventions for all community patients.
