Role Overview: Care Team Leader

Our Care Team Leaders provide direction, leadership, and hands-on support to our carers, ensuring that residents receive personalised care that meets their needs. They model the behaviours and values we hold in high regard, making sure our residents receive the best possible care.

Responsibilities
  • Manage a small team of staff, ensuring that they follow best practices
  • Conduct regular one-on-ones and appraisals to support and develop team members' career paths
  • Create initial personalised care plans
  • Administer medication and undertake daily observations of all residents
  • Identify changes in residents’ needs and take appropriate action
  • Ensure all records are up-to-date and care plans reflect residents’ evolving needs

About the Home

Part of a large charitable organisation, Connaught Court provides residential care, nursing and residential dementia support. We can support up to 94 residents with permanent care or short-stay respite breaks.
