Care Team Leader - Residential Care Home

Job Title: Team Leader

Location: Ruislip

Salary: £14.95 per hour

Hours: Minimum of 3 shifts per week (35.25 hours)

Shift Times:

  • Day: 7:30am - 7:45pm
  • Night: 7:30pm - 7:45am

Shift Flexibility: Candidate must be flexible to work both day and night shifts as required.

About the Role:

We are seeking an experienced and compassionate Team Leader to join our dedicated team at a residential care home supporting adults with learning disabilities, autism, and mental health conditions. This is a hands-on leadership role requiring a strong background in care planning, medication management, and team supervision.

Key Responsibilities:

  • Manage the full medication cycle: ordering, logging, administering, carry forward, replenishment, and auditing.
  • Develop, review, and update person-centred care plans using Person Centered Software (PCS).
  • Make referrals to and liaise with multidisciplinary professionals (GPs, Community Matron, District Nurses, Mental Health Teams, Dietitians, and Pharmacies).

Requirements:

  • Essential: Previous experience as a Team Leader in a residential care home setting.
  • Strong working knowledge of care plans and medication management.
  • Confident using Person Centered Software (PCS) or similar electronic care planning systems.
  • Excellent communication skills and fluent in written and spoken English.
  • Strong leadership skills with the ability to supervise, mentor, and guide staff.
