A leading healthcare provider in the United Kingdom is seeking a dedicated Care Coordinator for its Personalised Care Team. Join our award-winning SHaR PCN as a Care Coordinator specialising in Safeguarding, where you'll make a real difference to the lives of our most vulnerable patients across Ryedale and South Hambleton. This pivotal role sits within our well-established Personalised Care Team, focusing on coordinating care for children, families, and vulnerable adults who need safeguarding support. You'll work collaboratively with GPs, practice teams, and multi-disciplinary professionals to ensure timely, patient-centred care planning. This is an opportunity to enhance administrative processes for our most vulnerable patients whilst being the vital link between primary care, secondary services, and safeguarding agencies. SHaR PCN is an award-winning Primary Care Network comprising seven GP practices, serving the communities of Ryedale and South Hambleton. We're proud of our well-established Personalised Care Team, which includes Occupational Therapists, Health and Wellbeing Coaches, Social Prescribing Link Workers, and Care Coordinators.
Our recent growth has enabled us to develop two specialist work streams: 'Age Well' for over 65s and 'Live Well' for under 65s, allowing us to provide more targeted, personalised care. We're seeking a dedicated Care Coordinator to join our Personalised Care Team, specialising in safeguarding support for children, young people, families, and vulnerable adults. This role is integral to our mission of providing coordinated, patient-centred care whilst ensuring the safety and wellbeing of our most vulnerable patients.
As our Care Coordinator (Safeguarding), you'll be at the heart of ensuring our most vulnerable patients receive timely, coordinated care. You'll work across our seven GP practices, collaborating with an established multidisciplinary team to implement evidence-based safeguarding plans and improve patient journeys through primary and secondary care.
You'll be supported by a named clinical point of contact and work within our established governance frameworks. Regular attendance at quarterly safeguarding leads meetings will keep you connected with the wider safeguarding community and ensure you're up-to-date with best practices. We're committed to your professional growth. You'll undertake continual personal and professional development, participate in annual performance reviews, and have opportunities to contribute to service improvement initiatives. Training in MS Office and web‑based services will be provided as needed.

While based at Easingwold Health Centre, you'll be required to travel flexibly across all seven of our GP practices (Pickering, Kirkbymoorside, Helmsley, Terrington, Stillington, Tollerton, and Millfield) to deliver services and support patients in their local communities. This role therefore requires a current full driving licence and sole use of your own vehicle.
