- You will support and manage a care portfolio, including regular contact and client visits, so confidence in a client-facing role is essential.
- You will work closely with clients and carers to ensure a stable team of carers is rostered in advance and all parties are informed.
- You will conduct ongoing carer supervisions and annual appraisals for staff working with your clients.
- You should be adept at problem-solving and identifying issues to ensure continuity of care.
- All tasks should be performed with a mindset of delivering high-quality care and customer service excellence.
- As this role is supported by an On Call Care Coordinator team, it does not have on-call responsibilities and does not require shift coverage in the community.
