Overview

I'm delighted to be recruiting on behalf of a multi-award-winning homecare provider in Huddersfield. This is an excellent opportunity to join a supportive team of Care Coordinators, while working with a values-driven provider.

Benefits and package
  • £25,000-£29,000 salary OTE £31,000.
  • 28 days' annual leave.
  • Weekend on-call only (1 in 6 weekends at £145).
  • Team bonus (up to £1k per annum).
  • Opportunity for CPD and career progression.
Responsibilities
  • Managing and scheduling rotas for a team of 40 caregivers using digital systems (e.g. Access People Planner) to ensure consistent and effective care delivery.
  • Responding promptly to last-minute care changes or emergencies to minimise disruption for service users.
  • Communicating professionally with clients and their families to ensure needs are met and concerns are addressed.
  • Liaising with pharmacies to ensure timely and accurate medication delivery.
  • Supporting the Registered Manager with client assessments and risk assessments to maintain safe, person-centred care planning.
  • Building strong relationships with care staff to promote high standards of service.
  • Ensuring all documentation and records are accurate, up to date, and compliant with CQC regulations and internal policies.
Qualifications
  • Proven experience as a Care Coordinator or Field care Supervisor ready to step up within a homecare environment.
  • Strong competency with digital scheduling systems (e.g. Access People Planner).
  • Professional, reliable, and proactive approach.
  • Full UK Driving Licence.
