Job description

We are looking for an enthusiastic Community Nurse to join our Richmond Early Intervention Service (EIS). Applicants need to be professionally qualified and committed to providing responsive and high-quality clinical services to mental health service users and their carers.

The Early Intervention Team provides a service for people with first onset psychoses between the ages of 17 and 65 years. The aim of the service is to provide intensive care using the Assertive Community Team model, to prevent relapse, reduce the number of “revolving door” admissions, ensure effective treatment for resistant symptoms, reduce the incidence of suicide, and maximise an individual’s personal and social functioning.

The post holder will work within a multi-disciplinary team comprising of Doctors, Psychologists, Social Workers, CPNs, Occupational Therapists, Support and Recovery Workers, Employment Specialists, a Team Manager and team administration staff.

Role Summary

This role is for a Care Co‑ordinator/Key Worker to manage a caseload of clients aged between 18 and 75 who experience serious and enduring mental health illnesses, using evidence‑based, client‑centred principles to assess, plan, implement and evaluate interventions.

Responsibilities
  • Acquire up‑to‑date knowledge of treatments and protocols and apply it when working autonomously.
  • Work closely alongside patients and their carers as well as relevant agencies including GPs, acute hospitals, social services and voluntary support services.
  • Administer and monitor the side‑effects of medication, monitor service‑user compliance and report all incidents in line with trust policy.
  • Ensure the fundamentals of care are met – nutrition, hydration, hygiene, comfort, emotional and social support.
  • Coordinate and evaluate collaborative patient‑centred care, seek feedback and improve plans with the multi‑disciplinary team (MDT).
  • Provide accurate information about care in an accessible format to patients and their relatives/carers.
  • Liaise and work positively with members of the multi‑disciplinary team and partner organisations.
  • Support, teach and lead junior staff on risk assessment and risk management, making decisions based on research evidence, the individual service user and their social context, and clinical judgement.
  • Maintain safe custody of medicines, sharps and clinical equipment.
