Overview

An exciting opportunity has arisen to join the Urgent Community Response Norwich Team as a Care Co-Ordinator. We are seeking enthusiastic and proactive individuals to undertake administrative tasks to support the smooth and efficient running of the service. Urgent Community Response and Virtual Ward are operational all year round including bank holidays between the hours of 08:00 and 20:00. There may be a requirement to work some weekends and bank holidays.

Responsibilities
  • Coordinate patient appointments for the Urgent Community Response Team following set processes, onto live ledgers, using relevant data such as geography, skill mix and staffing levels.
  • Audit and monitor electronic information to ensure accuracy and quality.
  • Maintain good customer care when communicating with the public and health/social care professionals.
  • Triages referrals with health professionals to establish the appropriate destination and forward as necessary.
  • Demonstrate knowledge of automated facilities for receiving referrals, finding and checking patient information, and storing information in accordance with policies and procedures.
  • Produce clinical summaries, reports, letters, forms, notices and minutes of meetings as necessary.
  • Maintain information stored on the database by updating patient records daily and providing information as requested by team members.
  • Filing and storage of patient notes in accordance with policies and procedures.
  • Support and contact members of the intermediate care teams to ensure urgent referrals are dealt with within the appropriate response time by managing own time effectively and integrating this with team workings.
  • Be aware of and implement the Trust and departmental lone worker policy.
  • Receive and record messages for members of the teams.
  • Be an active team member with shared responsibility for general housekeeping, prioritising workload, contributing to the induction of new staff, and supporting the team.
  • Identify and communicate any problems regarding the service, including quality issues, and suggest improvements to the appropriate person.
  • Monitor and maintain stock levels, requisitioning and receipting as necessary with awareness of budgetary constraints.
  • Commit to working in a changing environment, respond to new demands, and be flexible to changing service needs.
  • Participate in the staff training programme, attend meetings, training sessions and external courses.
  • Engage in the staff appraisal scheme and maintain a dynamic personal development programme (PDP).
  • Demonstrate awareness of clinical governance and risk management and apply them to work situations.
