Main Duties of the Job
  • A patient care coordinator (PCC) may be required to deal with patients and, if appropriate, their carer, before or after the patient's consultation with a clinician or other healthcare professionals.
  • To work closely with practice and PCN healthcare roles, the PCC is to identify and work with a cohort of people to support their personalized care requirements, using our templates and software.
  • To collate all of a patient's identified care and support needs and review the options to meet these needs and bring them into a single personalized care and support plan (PCSP) in line with best practice.
  • To help people to manage their needs by answering their queries and supporting them in making appointments and accessing services.
About Us

We are a friendly, enthusiastic training practice that supports 17,800 patients, across two sites. We have a team of GP's, Nurses, Paramedics, Care Coordinators, and social prescribers with associated Mental health and Midwife services, working to support our community.

Job Responsibilities
  • To work closely with practice and PCN healthcare roles, the PCC is to identify and work via practice database with a cohort of people to support their personalized care requirements.
  • To collate all of a patient's identified care and support needs and review the options to meet these needs and bring them into a single personalized care and support plan (PCSP) in line with best practice.
  • To help people to manage their needs by answering their queries and supporting them in making appointments.
  • To assist patients to be better prepared to have conversations on shared decision making and to improve awareness of shared decision making and related support tools.
  • To provide patients with high-quality, easy-to-understand information to assist them in making choices about their care.
  • To support patients in understanding their level of knowledge, skills, and confidence (known as activation level) when participating in their health and well-being using, where appropriate, the PAM.
  • To liaise with other PCCs in other practices within the PCN and share best practice.
  • To assist patients to access self-management education courses, peer support, or interventions that support them in their health and well-being.
  • To provide coordination and navigation of patients, and where appropriate their carers, across health and social care services, where appropriate working hand in hand with social prescribing link workers (SPLW).
  • To support in the delivery of enhanced services and other service requirements on behalf of the PCN.
  • To attend and participate in the delivery of multi-disciplinary teams (MDT) within PCNs.
  • To develop yourself and the role through participation in training and service redesign activities.
  • To contribute to public health campaigns (e.g., flu clinics) through advice or direct care.
  • Undertake new patient health checks.
  • Support the practice nurse with health promotion programmes.
  • Carry out baseline observations such as pulse oximetry, blood pressure, temperature, and pulse rate, recording findings accurately.
  • Facilitate routine and 24-hour BP monitoring, advising patients accordingly.
  • Undertake wound care, dressings, and other clinical tasks as required.
  • Support the practice nurse with the management of chronic disease clinics.
  • Carry out BMI checks as directed.
  • Act as a chaperone as required.
  • When trained, undertake venepuncture.
  • When trained, administer flu vaccinations.
  • Carry out ECGs as requested.
  • Ensure specimens are recorded and ready for onward transportation.
  • Ensure all clinical rooms are adequately stocked and prepared for each session.
  • Ensure fridges are cleaned routinely in accordance with extant guidance.
  • Ensure clinical waste is removed from clinical areas and sharps bins replaced in accordance with the organization's IPC policy.
  • Deliver opportunistic health promotion where appropriate.

  • Educated to GCSE level or equivalent.
  • Experience of working in a healthcare setting.
  • Excellent communication skills (written and oral).
  • Good IT skills.

  • Good knowledge of MS Office and Outlook.
