Overview

This post is for an enthusiastic, dynamic clinician who wants to be involved in a novel and innovative approach to delivering cardiovascular services. The post will be based at Liverpool Heart & Chest Hospital NHS Foundation Trust (LHCH) and will be centred on the development and delivery of a community-based cardiovascular diagnostic & assessment service to the Knowsley, Merseyside population. There will be an option for a subspecialty session per week, if appropriate. The service aims to commission a single, cohesive provider of community-based cardiovascular services (CCVS) to improve health outcomes in a patient-focused manner, with emphasis on equality of access and reducing unnecessary secondary care use. The post works with Dr Clare Hammond, consultant cardiologist, and cardiology colleagues to deliver clinical quality and develop innovative cardiac rehabilitation and heart failure services within localities.

  • Knowsley CCVS – 4 direct clinical sessions (community clinics). 3 sessions fixed; 4th session flexible to cover annual/study leave.

Job description

The post is flexible in its nature and structure; full-time, part-time, and job-share applications will be considered. The successful applicant(s) will be based at LHCH and will work with Dr Clare Hammond to organise, develop and deliver a CCVS for Knowsley. Community-based clinics began in early April 2010 and operate in Kirkby, Huyton and Whiston. Eight clinical, consultant-led sessions are delivered per week, supported by ECG technicians and echocardiography-capable physiologists. The successful applicant will provide high-quality patient care in the community, request appropriate non-invasive investigations, and produce concise clinical diagnoses and management plans for referring clinicians. The postholder will have access to LHCH diagnostic services as needed, with existing administrative support and technician-led reporting that can be expanded.

Other support includes: full secretarial support, a personal computer, and office accommodation. The Trust supports continuing professional development with time and financial support and ensures annual appraisal and revalidation are in place, plus mentorship for newly appointed consultants.

Research, Audit, Teaching & Training

RESEARCH & AUDIT – Equivalent to one half-day per month. LHCH has a strong research and audit tradition. The successful applicant will ideally have research experience and be encouraged to contribute to the Centre’s research profile. Data collection for CCVS will support epidemiological studies and other clinical research, with key performance indicators based on clinical quality and regular dissemination of findings via audit meetings.

TEACHING & TRAINING – The post-holder will promote CCVS awareness among community clinical staff serving Knowsley. Opportunities exist to establish formal GP cardiovascular training with ICB collaboration, in community and LHCH settings. LHCH supports specialist-nurse delivered services and the post-holder is expected to contribute to education and training.

Person Specification

Qualifications

  • Essential: MRCP (UK) or equivalent CCT (or within 6 months) in cardiology or general medicine
  • Desirable: Higher degree (MD, DM, DPhil, PhD)

Management

  • Essential: Demonstrates leadership and ability to contribute to service structure/delivery; understands team-based work
  • Desirable: NHS management conferences, committee work, experience with CCGs and hospital Trust management

General Cardiology

  • Essential: Registrar or consultant-level experience in outpatient cardiovascular assessment, including interpretation of non-invasive tests
  • Desirable: British Society of Echocardiography accreditation; ALS provider or instructor

Education / Training

  • Essential: Teaching experience in clinical settings; evidence of teaching activities and feedback
  • Desirable: Level 1 and Level 2 accreditation as an educational supervisor; attendance at teaching skills courses

Research & Audit

  • Essential: Evidence of published or presented cardiovascular research and audit activity
  • Desirable: Publications in cardiovascular epidemiology/ prevention; research degree; successful grant applications
