Overview
An opportunity has arisen for a MDT Co-ordinator/MDT Tracker to work within the Multi-Disciplinary Team, coordinating and tracking patients through their cancer pathway to provide high quality care.
The Cancer Services Directorate provides pathway coordination and tracking for patients with known and suspected cancer. We are responsible for submissions to Cancer Waiting Times, The Cancer Registry, Peer Review and National cancer audits and datasets. Confidence in operating IT systems is essential.
Responsibilities
- Coordinate and track the cancer patient tracking list (PTL) and organise MDT meetings.
- Prepare appropriate lists of patients to be discussed at each MDT from information from clinicians, medical notes, electronic patient records and previous MDT lists; distribute these lists to the team.
- Prepare MDT decision sheets prior to MDT meetings with patient demographics, investigation results and other relevant clinical information.
- Ensure MDTs are informed of progress regarding 31 and 62 day target dates for treatment, coordinating with MDT meetings and other stakeholders to manage the patient journey proactively.
- Liaise with Patient Pathway Coordinators and other departments (e.g., imaging, histopathology, chemotherapy, palliative care, other trusts) to ensure required information and clinical data are available for discussion in a timely manner.
- Book and confirm decisions requiring administrative actions (e.g., scans, appointments, surgery dates) and follow up with relevant departments and specialists to ensure timely arrangements.
- Assist with follow-up actions required by the MDT (e.g., contacting patients to arrange the next part of their pathway).
- Ensure MDT decisions are documented in patient notes and file the MDT decision summary and related documents in case notes promptly; retain copies for future use and audit purposes.
- Enter all MDT decision information into the Somerset Cancer Register system (during or after the MDT); liaise with clinicians to ensure accuracy and timeliness.
- Ensure audio/visual equipment is available for recording MDT decisions on the Somerset Cancer Register system.
- Organise Specialist Multi-Disciplinary Team (SMDT) meetings in the same manner as MDT meetings; inform clinicians at other Trusts across London Cancer of changes to meeting time/venue and any SMDT format changes as appropriate.
