Job description

We are seeking a dedicated and compassionate Healthcare Professional to join our team in Southampton, Hampshire. In this role, you will provide high-quality care and support to our adult female client ensuring their comfort, safety, and well-being with active participation in goal-directed rehabilitation interventions as well as helping her with her executive functions.

It is essential that you drive for this position.

Working Hours:

Friday: 10:30 - 18:00

Sunday: 10:00 - 18:00

Key Responsibilities:

  • This will include supporting the client in functional daily living activities, supporting with organisation, and planning, and supporting her communication needs, both spoken and written.
  • She willneed help to draw up realistic plans and schedules, prompts to record and check information, and help to get things done.
  • Thinking supports are also needed, including support to link information together to make sound decisions and to solve problems.
  • Her rehabilitation is currently co-ordinated by a case manager with input from a multi-disciplinary team to address on-going physical, cognitive, and linguistic processing difficulties.

Requirements:

  • Previous experience in a similar role, preferably in complex care.
