What you will do
  • Clinically screen medication charts on the ward, ensuring patient safety and effective treatment plans
  • Collaborate with multidisciplinary teams to provide ward-based medicine information and address patient queries
  • Screen and process discharge prescriptions and one-stop orders for quality and accuracy
  • Conduct medicines reconciliation and interpret blood results in relation to medications
  • Perform clinical checking and conduct ward-controlled drug audits, ensuring compliance and safety
  • Order medicines and work closely with Medicines Optimisation technicians to enhance patient outcomes
Requirements
  • Must be a registered pharmacist with the HCPC
  • A valid driver’s licence is required as travel within the city or between healthcare facilities may be necessary
  • Familiarity with S1, SeeEHR, ICM, and pharmacy dispensing systems is highly desirable, alongside proficiency in standard Microsoft applications
