A leading NHS mental health service provider in Bristol is seeking a professional to assist young people transitioning from children's to adult mental health services. The successful candidate will ensure a smooth transition, provide expert advice, and support young individuals and their carers. Working with young people experiencing poor mental health across Bristol, who are in secondary mental health services and helping them and their mental health professionals in mapping and transitioning into a pathway to manage their mental health beyond their 18th birthday. The post holder will ensure a cohesive, smooth transition for young adults from Children and Adolescent Mental Health Services (CAMHS) to Adult Mental Health Services (AMHS) that is evidenced as being significant in terms of their longer term recovery. They will also provide expert advice and support to all relevant stakeholders with regards to young adults who are receiving support from secondary mental health services who are approaching their 18th birthday. NICE guidelines detail good practice in respect to the transition process and will help inform future practice.
