Overview

Working with young people experiencing poor mental health across Bristol, who are in secondary mental health services and helping them and their mental health professionals in mapping and transitioning into a pathway to manage their mental health beyond their 18th birthday. The post holder will ensure a cohesive, smooth transition for young adults from Children and Adolescent Mental Health Services (CAMHS) to Adult Mental Health Services (AMHS) that is evidenced as being significant in terms of their longer term recovery. They will also provide expert advice and support to all relevant stakeholders with regards to young adults who are receiving support from secondary mental health services who are approaching their 18th birthday. NICE guidelines detail good practice in respect to the transition process and will help inform future practice.

Key Responsibilities
  • Work as a member of the South Gloucestershire adult recovery service and link into, and share a base with, the Child and Adolescent Mental Health Services.
  • Provide comprehensive and specialist advice around the development of a care pathway for a young person experiencing emotional dysregulation who is transitioning into AMHS.
  • Work with professionals and carers for the young person to ensure transition is seamless.
  • Plan, prioritise and work alongside the young person to ensure the transition process is clearly understood and managed effectively.
  • Ensure that a care coordinator is identified within AMHS and that joint working is undertaken during the period of transition.
  • Work with and on behalf of young people in a way that promotes and develops their efforts, resources and quality of life, delivering services in a manner that is appropriate, relevant and respectful to all community members and recognising diversity of ethnicity, culture, belief, privilege and capacity.
  • Liaise and work in partnership with staff from other statutory services, voluntary agencies and community groups that work with adolescents.
  • Provide advice and assist other professionals working with young people with interventions that draw on own specialist professional knowledge.
  • Provide advice and support to carers/relatives of young people.
  • Participate in clinical meetings such as CPA and client reviews as appropriate.
  • Support the full implementation of the Care Programme Approach.
  • Ensure that legal and Trust requirements affecting service users and staff are adhered to, and maintain a safe working environment for service users and staff. Cooperate with statutory agencies with the responsibility to protect children and ensure risk and safety issues are assessed and necessary action is implemented.
  • Maintain a high degree of professionalism at all times when working with highly distressed people.
  • Receive regular clinical supervision in line with Trust policy, and also participate in caseload supervision arrangements provided by the Wiltshire Early Interventions for Psychosis Service.
  • Maintain accurate records relating to clinical and professional activity, and monitor clinical workload using agreed systems.
  • Communicate complex and sensitive information to patients, carers and professionals, including information about diagnosis and prognosis, enabling young people and their families to understand this information and providing empathy, reassurance and hopefulness.
