Job Title: Band 6 Occupational Therapist – Acute Medical Team
Location: Chichester, Sussex, UK
Job Purpose: To provide high-quality occupational therapy input within the Acute Medical Team. To manage patients with complex health and social needs, supporting safe discharge and recovery. To work autonomously while contributing to a collaborative multidisciplinary team.
Key Responsibilities
- Independently manage a caseload of patients with complex medical and social needs using evidence-based practice
- Carry out comprehensive assessments and implement effective treatment programmes
- Apply strong clinical reasoning and analytical skills, adapting interventions as patients’ needs change
- Assess patients’ capacity and ability to give informed consent for therapy
- Actively involve patients in their treatment, ensuring informed consent is obtained at all times
- Complete cognitive and mood assessments using recognised standardised tools
- Organise and undertake home visits in line with home visit guidelines
- Communicate outcomes clearly through written and verbal reports to internal teams and external agencies
- Support effective discharge planning to ensure safe and timely transitions of care
- Empower patients through education, advice, coping strategies, and therapeutic interventions
- Build effective therapeutic relationships with patients presenting cognitive, perceptual, or behavioural challenges
- Maintain accurate, timely documentation in accordance with Trust, departmental, and professional standards
Essential Requirements
- HCPC registration as an Occupational Therapist
- Experience working within an acute medical setting
- Strong clinical reasoning, analytical, and decision-making skills
- Ability to work independently and manage complex caseloads
- Excellent communication and interpersonal skills
