Beacon House is seeking a compassionate and motivated Band 5 Registered Nurse to join our nurse‑led clinic supporting people experiencing homelessness and severe social exclusion. The successful candidate will work as part of a multidisciplinary team to support Guests with complex physical health needs, health inequalities, and barriers to accessing mainstream healthcare.
Deliver safe, effective, and evidence‑based nursing care within the nurse‑led clinic
Undertake clinical assessments, observations, wound care, and health checks
Support long‑term condition management (e.g. respiratory disease, cardiovascular risk, infections)
Administer vaccinations and medicines in line with PGDs and local protocols
Provide smoking cessation and health promotion interventions
Maintain accurate, timely clinical records using SystmOne
Identify safeguarding concerns and elevate appropriately
Work collaboratively with GPs, nurses, outreach teams, housing providers, and voluntary agencies
Support engagement of guests who may be anxious, disengaged, or distrustful of services
Contribute to service development, audit, and quality improvement
Maintain professional standards in line with the NMC Code of Conduct

Main Duties of the Job
Deliver safe, effective, and evidence‑based nursing care within a nurse‑led clinic setting
Undertake holistic clinical assessments, observations, wound care, and routine health checks
Support the management of long‑term conditions, including respiratory disease, cardiovascular risk, and infections
Administer vaccinations and medicines in accordance with PGDs, Patient Specific Directions, and local protocols
Provide smoking cessation support and health promotion interventions
Identify and elevate safeguarding concerns in line with local and organisational policies
Support engagement with guests who may be anxious, disengaged, or distrustful of healthcare services
Work collaboratively with GPs, nurses, outreach teams, housing providers, and voluntary sector partners
Maintain accurate, timely, and confidential clinical records using SystmOne
Contribute to service development, audit activity, and quality improvement initiatives
Maintain professional standards in line with the NMC Code of Conduct
Registered Nurse (Adult) with valid NMC registration
Experience working in primary care, community nursing, or acute healthcare settings
Excellent communication and interpersonal skills
Ability to work independently and as part of a small multidisciplinary team
Commitment to equality, dignity, and trauma‑informed care
Confident using electronic patient record systems
Experience working with inclusion health groups or vulnerable adults
Experience delivering vaccinations and/or smoking cessation services
Knowledge of adult safeguarding and health inequalities
Experience working in nurse‑led clinics, outreach, or non‑traditional healthcare settings
