A local healthcare provider in England seeks a full-time pharmacist to join their team. Salary - Band 7-8aequivalent pro rata, however as not Agenda for Change salary will be based onprevious experience

Undertakestructured medication reviews for patients both in and out of care homes toproactively manage patients with chronic diseases and complex polypharmacy
Supportmanagement of medicines on transfer of care, especially back into primary care
Supportthe development and implementation of systems for safer prescribing andmonitoring of medicines
Providespecialist medicines management advice to patients and health careprofessionals
Workwith community and hospital colleagues to help resolve medication-relatedissues
Workon, and lead on, large scale projects to improve medicines safety across thePCN
AvonValley Primary Care Network is an umbrella organisation delivering care to40,000 patients, across four GMS Practices based in the New Forest area. ThePractices include The Fordingbridge Surgery in Fordingbridge, The RingwoodMedical Centre and Cornerways Surgeries in Ringwood and Twin Oaks Surgery inBransgore. The practices have been working together for some time and have agood relationship. Together we aim to build robust services across our networkto improve patient care and are keen to bring in new members of the primarycare team to achieve these goals.
Tobe part of a multi-skilled primary care team in a patient facing role providingpatient centred medical services based on patient needs with a focus oncontinuity of care.
