This pivotal role involves providing essential direction, guidance, and support to our care teams, ensuring seamless, high-quality care interventions for our residents.

As an Assistant Practitioner, you will play a crucial role in enhancing our residents' well-being through your advanced training and clinical expertise. You'll work closely with Registered Nurses and Unit Managers, contributing to both team development and direct resident care.

Key Responsibilities Include:
  • Direct Care & Support:Delivering best practice care, assisting residents with daily living, reviewing care plans, and supporting social activities.
  • Clinical Interventions:Performing trained clinical tasks such as medication administration (including injections, insulin), oxygen administration, catheter care, PEG management, wound care, and more.
  • Team Leadership:Providing clear direction, guidance, and support to Health Care Assistants, fostering development, and educating care teams, residents, and families.
  • Quality Assurance:Contributing to risk assessments, ensuring accurate record-keeping and communication, and maintaining high standards of care.
  • Compliance:Adhering to all Oakland Care policies, including safeguarding, health and safety, and promoting safe working practices.
What We're Looking For:
Essential Qualifications, Knowledge & Experience:
  • Substantial and demonstrable experience in delivering care.
  • Level 3 in Health and Social Care
  • Successful completion of the Oakland Care Assistant Practitioner training course or evidence of an equivalent recognised qualification.
  • Evidence of competence and certification of all clinical skills required for this role.
  • Completion and competent in medication administration either Guardian Angels medication training or equivalent.
  • Experience in team member induction and supervision.
