Job description

We are looking for an enthusiastic, dynamic and caring individual who is passionate about Stroke and neuro rehabilitation. If you are keen to work in the community and make a difference in people’s lives with rehabilitation, then this post is for you.

The post holder will work autonomously and closely with other therapists within the neurotherapy team, supported by a multidisciplinary team including physiotherapists, occupational therapists, speech and language therapists, psychologists and other assistant practitioners.

Nature of work

The post holder will be part of a multidisciplinary therapy team including physiotherapists, occupational therapists, speech & language therapists, clinical psychologists and other assistant practitioners.

The post holder will be responsible for delivering direct clinical care and delegated therapy interventions for service users referred to the Neurotherapy service.

Key responsibilities include:

  • Involving patients and carers in the planning of care/rehabilitation programmes and encouraging self‑management where appropriate.
  • Communicating effectively with all members of the Neurotherapy service and the wider Bedfordshire Community Health Services.
  • Supporting and promoting teamwork generally.
  • Maintaining accurate patient records, including patient held records.
  • Providing written reports and verbal feedback to the team regarding client performance and progress in areas of self‑maintenance, productivity and leisure.
  • Acting as the named person for the client to contact within the department and feeding relevant issues to senior staff.
  • Having a basic level of knowledge of support services within the community.
Planning
  • Planning and implementing daily work within the team and across a wide geographical area, adapting to unpredictable changes in service demands.
  • Participating in team planning by organising new referrals and managing ongoing work allocations, enabling effective and efficient team functioning.
  • Supporting other team members in planning and managing their caseloads, especially in the event of absence.
  • Attending caseload meetings and multidisciplinary therapy meetings, ensuring information on individual patients is fed back to the team for planning purposes.
  • Prioritising and managing own workload, handling conflicting demands on your time.
Patient contact
  • Promoting the team philosophy that embraces a holistic approach to patients, carers and relatives.
  • Carrying out initial screening and assessment of clients on behalf of the multidisciplinary therapy team.
  • Reviewing and re‑assessing delegated interventions, keeping relevant clinical staff informed of any progress or concern.
  • Undertaking clinical procedures and therapeutic activities within a goal‑oriented programme under direction from a variety of multidisciplinary team members.
  • Providing all relevant information regarding clients to appropriate clinicians/therapists, including any changes in condition and circumstances.
  • Fitting and demonstrating equipment to clients and their carers.
  • Ordering equipment from the community equipment store and using the PIN number responsibly.
  • Advocating for patients, making team members aware of any concerns if it is in the patient’s best interest.
  • Identifying areas of shared assessment with other team members and liaising with other agencies to refer patients accordingly.
  • Actively promoting good health and development and participating in public health initiatives.
  • Carrying out other clinical duties under instruction from senior staff as required across localities to meet patient needs.
  • Safely traveling between patients’ homes and other venues, adhering to relevant policy and guidelines.
  • Regularly undertaking moving, positioning and mobilisation of patients and equipment in line with Trust Moving and Handling Policy.
  • Engaging in patient handling and mobility tasks, including transfers, gait training, passive exercises and use of hoists and other equipment, supporting patients with significant physical impairments.
  • Demonstrating and advising on a range of equipment to clients and relatives.
  • Working within IT systems and maintaining adequate keyboard skills.
  • Developing and delivering enhanced skills in a specific therapy domain.
Communication
  • Communicating complex, sensitive information to a wide variety of professionals in a professional manner.
  • Providing updates and reports—written and verbal—related to service users’ needs and care.
  • Working within Trust policy on communication of clinical risk and information sharing.
  • Maintaining confidentiality and understanding issues surrounding it.
Important relationships job holder must maintain
  • Internal: Leadership team, all members of the Neurotherapy service, discharge planning team, specialist nurses (safeguarding leads, tissue viability, etc.).
  • External: Hospital nursing and therapy staff, patients, relatives and care agencies, local authorities including adult social care, general practitioners, emergency care practitioners, clinical commissioning groups.
People Management
  • Maintaining own professional development and participating in appraisal and KSF process.
  • Completing relevant stroke competencies as required for the post.
  • Developing own clinical knowledge and participating in the development of clinical knowledge for others.
  • Maintaining up‑to‑date skills and knowledge and an awareness of professional issues.
Research & Development Activity
  • Contributing to meetings about clinical governance and service development.
  • Participating in the delivery of the Neurotherapy service development plan.
