Overview
This is an exciting new ANP/ACP role, working for the Dow Surgery based in Redditch. The role will be predominately focused on same day, medically urgent patients.
Responsibilities
- Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
- Implement and evaluate individual specialised treatment plans for chronic disease patients
- Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health
- Provide advanced, specialist care to patients as required in accordance with clinical based evidence, NICE and the NSF
- Undertake the collection of pathological specimens
- Request pathology services as necessary
- Process and interpret pathology and other test results as required
- Maintain accurate clinical records in conjunction with extant legislation
- Ensure SNOMED CT codes are used effectively
- Chaperone patients where necessary
- Prioritise health issues and intervene appropriately
- Support the team in dealing with clinical emergencies
- Recognise, assess and refer patients presenting with mental health needs
- Be an extended and supplementary prescriber, adhering to guidance
- Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required
- Contribute to practice targets (QOF, etc.), complying with local and regional guidance
- Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)
- Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)
- Support the clinical team with all safeguarding matters in accordance with local and national policies
- Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
- Deliver opportunistic health promotion where appropriate
Experience
- Ability to evidence a sound understanding of the NHS principles and values.
- Ability to analyse and interpret complex/ often incomplete information, pre-empt and evaluate issues, and recommend an appropriate course of action to address the issues.

Qualifications
- ANP:
- Advanced Practitioner Nurses must have either:
- 1. graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centres ePortfolio (Supported) Route; both pathways enable eligibility for an Advanced digital badge issued by the Centre for Advancing Practice, demonstrating recognised educational and experiential preparation in advanced practice; or
- 2. enrolled as a trainee on a Centre for Advancing Practice accredited MSc advanced practice programme linked with subsequent guaranteed progression onto a reimbursable Advanced Practitioner Nurse role on completion of training; or
- 3. enrolled on and progressed past the initial Learning Needs Analysis (LNA) stage of the Centre for Advancing Practice ePortfolio (supported) Route, linked with subsequent guaranteed progression onto a reimbursable Advanced Practitioner Nurse role on completion of the ePortfolio process.
- ACP:
- Advanced Practitioner:
- a. has a masters degree level in the relevant area of expertise;
- b. is working at a masters level aware or equivalent that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competencies
- Mentorship or supervisory skills training.
