Job description
Anticoagulant Support Worker

The closing date is 10 December 2025

An opportunity has arisen for an experienced support worker to join an established team of anticoagulant nurses. You will help monitor patients who take Warfarin, working both autonomously and as part of a busy multidisciplinary team. Your role will involve point‑of‑care INR testing for patients within the clinical setting and in the community, as well as home visits for house‑bound patients across Heywood, Middleton and Rochdale. You will plan your own routes each day and follow established clinical and lone‑working protocols.

The position requires the ability to travel across the HMR footprint and prioritise workload. Excellent communication skills are essential – you will convey blood results to the anticoagulant nurse by telephone and leave written instructions for patients at home. You will liaise with community neighbourhood teams, rapid response teams and other care pathways to support patients in their homes.

High autonomy, caring and compassion are required to deliver quality care.

Main duties of the job

The post holder will assist the anticoagulant nurse in running hospital and community anticoagulant clinics and will carry out tasks delegated to them. Responsibilities include:

  • Conducting INR blood testing with the Coaguchek device, managing equipment and following daily quality control and standard operating procedures.
  • Planning and prioritising home visits to maximise time utilisation and fuel economy.
  • Maintaining communication with patients, relatives, carers and the multidisciplinary team to provide holistic patient care.
  • Working flexibly between primary and secondary care to facilitate specialist assessments for house‑bound Warfarin users.
