A healthcare provider in Greater Manchester is looking for an Anticoagulant Support Worker to monitor patients on Warfarin. The role involves conducting INR testing, assisting with clinics, and making home visits. Candidates must have NVQ Level 3 in Healthcare and strong communication skills. An opportunity has arisen for an experienced support worker to join an established team of anticoagulant nurses. You will help monitor patients who take Warfarin, working both autonomously and as part of a busy multidisciplinary team. Your role will involve point‑of‑care INR testing for patients within the clinical setting and in the community, as well as home visits for house‑bound patients across Heywood, Middleton and Rochdale. You will plan your own routes each day and follow established clinical and lone‑working protocols.
The position requires the ability to travel across the HMR footprint and prioritise workload. Excellent communication skills are essential – you will convey blood results to the anticoagulant nurse by telephone and leave written instructions for patients at home. You will liaise with community neighbourhood teams, rapid response teams and other care pathways to support patients in their homes.
High autonomy, caring and compassion are required to deliver quality care.
