Overview

Advanced Practitioners work closely with their team, health and community colleagues to gain an in-depth understanding of community resources that facilitate and build independence, maintaining people's ability to remain living in their own community networks and working with a strengths-based approach. We encourage applications from a diverse range of backgrounds and experiences.

Responsibilities
  • Work in the office base a minimum of 2 days per week, with the rest of the week hybrid.
  • Utilise your extensive skills and experience to guide your team to support and empower residents of Somerset to remain independent for longer in their communities.
  • Tailor solutions around residents’ needs and provide continuity of support and intervention.
  • Promote independence across all team activities.
  • Collaborate with Health and Social Care colleagues as part of the neighbourhood PCN’s.
  • Liaise with key partners to improve service provision, team training and recruitment.
  • Contribute to the development, shaping and building of integrated care local services.
  • Supervise registered Social Workers and Adult Social Care Practitioners within your neighbourhood.
Qualifications
  • Qualified Social Worker and registered with Social Work England.
  • Full access to or use of a vehicle is essential (required for travel across rural Somerset).
  • Experience in leading or managing teams is desirable.
