Advanced Nurse Practitioner - Home Visiting Service

Salary Range:Dependant on experience and qualifications - up to FTE£51,700 pa - (Pro-rata £31,020 pa)

Norfolk Primary Care (NPC) is an established Primary Care provider of clinicalservices. We are a key provider ofPrimary Care services in the Norfolk & Waveney system workingwith the local GP practices.

NPC has been sub-contracted to provide a home visitingservice to the Practices in the Norwich PCNs. This comprises of a team ofclinicians who are GPs, Advanced NursePractitioners, qualified nurses with a prescribing qualification (orworking towards), and Advanced Clinical Practitioners working across theNorwich PCNs. The staff provides additional Primary Care support to thatcurrently being provided by individual practices.

Supported by a Service and Clinical Lead, the HVS team areresponsible for working with all HVSparticipating Practices focusing on proactive support to patients residing athome (including care homes).

The aim of the HVS is to reduce the number of avoidableadmissions into hospital and todeliver more consistent, enhanced services from Primary Care. Itoffers proactive health care support tohousebound patients, alongside the general population of patients who mayrequire home visiting services.

Main duties of the job

This exciting role will provide ahigh quality service to housebound patients on behalf of their registered GPPractice, and the wider PCN.

You will be an experiencedclinician who will be acting within their professional boundaries andcompetencies. You will provide care forthe presenting patient from initial history taking, clinical assessment, diagnosis,treatment, referral (where required), evaluation, and follow up of care.

You will demonstrate safe,clinical decision-making and expert care, including assessment and diagnosticskills, for the patients within Primary Care. The post holder will demonstratecritical thinking in the clinical decision-making process. They will workcollaboratively with the GP Practicesto meet the needs of patients in supporting the delivery of policy and procedures, and providingnurse leadership if / as required.

In order to work at this level,those registered with the NMC / HCPC, requirements for Advanced Practice mustbe met.

Job responsibilities
  • Clinically examine and assess patient needs froma physiological and psychological perspective and plan clinical careaccordingly.
  • Assess, diagnosis, plan, implement and evaluateinterventions/treatments for patients with complex needs. Proactively identify,diagnose and manage treatment plans for patients at risk of developing along-term condition (as appropriate).
  • Refer patients for investigation or specialistopinion, where appropriate.
  • Diagnose and manage both acute and chronicconditions, integrating both drug and non-drug based treatment methods into amanagement plan.
  • Work with patients in order to supportcompliance with and adherence to prescribed treatments.
  • Ensure provision of continuity of care includingfollow up appropriately.
  • Take guidance and instruction from theregistered GP, when and if required.
  • Where the post holder is an independentprescriber: to prescribe safe, effective and appropriate medication as definedby current legislative framework.
  • Provide information and advice on prescribed orover-the-counter medication on medication regimes, side-effects andinteractions.
  • Prioritise health problems and interveneappropriately to assist the patient in complex, urgent or emergency situations,including initiation of effective emergency care.
  • Refer patients to other health careprofessionals as appropriate for clinical needs and in accordance with localframework/guidelines if indicated.
  • Meet the needs of patients presenting foropportunistic long term condition management or assessment.
  • Be able to undertake medication reviews forindividuals on any type of medicationswithin an agreed skill level.
  • Verification of expected death (whereappropriate training and guidance allows).
  • End of life - to support advanced care planning.
  • Be able to undertake general telephone triage /first contact assessment.
  • To manage own clinical workload within the HomeVisiting Service, responding effectively to patient need.
  • Work within the latest NMC / HCPC Code ofProfessional Conduct at all times.

Safeguarding Childrenand Vulnerable adults

  • All staff are expected to behave in such a waythat supports the commitment of NPC to safeguard and promote the welfare ofchildren, young adults, and vulnerable adults.
  • You will be responsible for safeguarding theinterests of children and adults who you come into contact during your work. Tofulfil these duties, you will be required to attend training and development torecognise the signs and symptoms of abuse or individuals at risk, to followlocal and national policy relating to safeguarding practice and to report andact on concerns you may have appropriately.

Other duties

  • Any otherduties, as agreed with the Service or Clinical Lead to meet the needs of theorganisation, mayinvolve travel to other sites within the organisation.
  • You must holda full drivers licence with access to a vehicle to use for purposes of travelto and from Practices and to carry out home visits. This will involve havingthe appropriate insurance to use your vehicle for business use.

Qualifications
  • Registered first level nurse / BSc (Hons) or MSc Advanced Practice (or equivalent), or HCPC registration, whichever is appropriate.
