A health organization in the UK is seeking an experienced Advanced Nurse Practitioner to join their team. The role involves assessing and managing patients with chronic conditions, providing education and support to patients, and collaborating with a multidisciplinary team. We have a great opportunity for an enthusiastic, highly motivated Advanced Nurse Practitioner to join our friendly and supportive team. Working in collaboration with other members of our multi-disciplinary team you will bring experience in assessment, diagnosis, treatment and management of both acute and chronic conditions. As an independent prescriber you will support our patients with prescribing, issuing and reviewing medications as appropriate.

Many clinical staff develop leadership roles for areas of interest and are supported in their development. We are also keen to invest in the future workforce and as a training practice provide learning and development to trainees and students, some of whom join us permanently following qualification.
We are part of Community Care PCN comprised of 3 local practices and our large cohort of additional roles staff support and share the delivery of clinical care ensuring our patients see the right clinician for their needs. Our dedicated, multi-disciplinary team are passionate about providing excellent care for our patients and the successful candidate will be committed to delivering safe clinical practice working in partnership with our patients.
Main duties of the jobWe are a large, innovative GMS Practice based in Hall Green Birmingham with an increasing patient list of 31,500 patients. We have a good reputation in our locality and with patients, and a strong focus on patient care.
Our extensive clinical team incorporates 9 GP Partners and 17 Salaried GPs, plus a full range of allied health professionals including Nursing, HCAs, ANPs, Paramedic, PAs, Clinical Pharmacists and Physiotherapists, supported by a large non-clinical team providing a full range of reception, administration and management functions. As part of Community Care Hall Green PCN we are further supported by a range of clinical and non-clinical AARs staff.
We work together collaboratively, continually look for ways to improve our services and actively support the wellbeing of one another. Our services have a good CQC rating with outstanding well led features and we regularly achieve high QOF performance. We continue to be at the forefront of digital healthcare innovation and actively engage with our patients and PPG to expand, improve and trial new service initiatives.
