Overview

Little London Surgery is a busy, friendly GP practice serving a diverse population. We are looking for an experienced Advanced Nurse Practitioner (ANP) to join our clinical team and help deliver safe, high-quality care for our patients.

We are part of South 1 Primary Care Network (PCN) a collaborative group of 8 other member practices, offering shared learning, MDT, shared workforce.

• Independent assessment, diagnosis, and management of patients
• Same day/urgent care and routine appointments (depending on service need)

• Working closely with GPs, nurses, pharmacists, paramedics, and the wider team

Main duties of the job

Job purpose

To provide advanced clinical care, including assessment, diagnosis, treatment, prescribing, and referral for patients presenting with acute, urgent, and routine primary care needs. The ANP will work autonomously and as part of the wider MDT.

Key responsibilities

Clinical

• Undertake face-to-face and telephone consultations with patients of all ages where appropriate
• Assess, diagnose, treat and manage patients within scope of ANP practice
• Request, interpret and act upon investigations
• Prescribe medications and/or treatment plans in line with qualification, PGDs, and best practice
• Refer appropriately to secondary care and community services
• Provide safety-netting and clear documentation for all consultations
• Recognise deteriorating patients and escalate appropriately
• Participate in duty/urgent care (if required by rota)

Long-term conditions (if within skillset)

• Support management of chronic conditions (e.g., asthma, COPD, diabetes, hypertension)
• Contribute to preventative care: immunisations, screening uptake, health promotion

Person Specification

Qualifications

• Registered Nurse with current NMC registration
• Advanced clinical assessment skills with evidence of autonomous practice
• Experience working in General Practice
• Excellent communication and consultation skills
• Ability to work independently and manage risk appropriately
• Strong understanding of safeguarding, confidentiality and documentation standards
• NMC registration
• Evidence of advanced practice training/competence
• Level 7 in Advanced Clinical Practice
• MSc Advanced Clinical Practice (or equivalent)
• Qualified to Level 7 in Advanced Clinical Practice
• MSc/PGDip/PGCert in Advanced Clinical Practice or working towards
• Experience managing minor illness / urgent presentations
• Experience with frailty, acute presentations, womens health, or paediatrics depending on need
• Experience contributing to audits and quality improvement
