Job Advert: Advanced Nurse Practitioner – Uxbridge

A forward-thinking, patient-centred GP practice in Uxbridge is seeking an experienced Advanced Nurse Practitioner (ANP) to join its established clinical team.

The practice is a modern, well-regarded training environment with a strong reputation for high QOF achievement. It supports both registrars and medical students, and offers a collaborative working culture. The team currently includes GPs, ANPs, practice nurses, HCAs, and a clinical pharmacist, with additional support from PCN staff such as a pharmacist, pharmacy technician, physiotherapist, social prescriber, mental health nurse, and community matron.

The Role
  • Assess, diagnose, plan and implement care for patients with both acute and long-term conditions
  • Provide high-quality consultations, with the ability to prescribe independently
  • Undertake health screening, treatment services, and patient education
  • Deliver immunisations, travel vaccinations, and chronic disease management
  • Work collaboratively with colleagues to ensure safe, effective, and evidence-based care
  • Contribute to the ongoing development of the practice through participation in clinical meetings, audits, and service improvement
