We have an opportunity for an Advanced Nurse Practitioner to workas part of our excellent and friendly clinical team in supporting the duty/sameday service and management of long term conditions.

We want a caring healthcare professional who takes a pride indelivering a service that is of a high standard.

The post is initially for two days a week either a Tuesday,Thursday or Friday. The hourly rate will be dependent on experience.

As a member of the team, you have access to the NHS pensionscheme for which the employer contributions are 23.7% and the Blue Light card,offering a wide range of exclusive discounts to public sector workers. Afull-time post would have 35 days holiday, including bank holidays (pro-ratafor part-time employees).

Main duties of the job

We are looking for an advanced nurse practitioner to workautonomously in practice within the practice team to support the duty/same dayservice as well as providing care for patients with long term conditions.

You will be an integral part of both our practice and the widermultidisciplinary teams operating across primary care.

Candidates who are qualified asadvanced care practitioners but are not yet qualified as an independentprescriber must be prepared to work towards a non-medical prescribingqualification.

Job responsibilities

Our Advanced NursePractitioner has the following key responsibilities in delivering healthservices to our patients:

  • Practice both independently withinscope of practice and in partnership with the multidisciplinary team (MDT) underthe clinical supervision of a General Practitioner, ensuring interventions andtreatments are undertaken in line with best practice.
  • Work as part of the practiceand wider multidisciplinary teams to support the delivery of the duty/same dayservice and reviewing complex patients with long term conditions and sharingexpertise to improve patient care.
  • Be an independent prescriber, usingprescribing and deprescribing skills to optimise the use of medicines toimprove patient outcomes.
  • Undertake home visits for thepractice.
  • Perform at an advanced levelunder the core capabilities across the four pillars of Advanced Practice(Health Education England Multi-Professional Framework 2017):

  • Undertake clinical assessmentof the patient, analysing complex clinical signs, patient history, andinvestigation to establish a clear care plan, including the generation ofanticipatory, escalation, or admission avoidance plans, arranging rescuemedicines and ensure the completion of out of hours special messages.
  • Interpret clinical results andact on findings accordingly.
  • Admit and discharge patientsfrom the caseload appropriately, providing accurate and effective clinicalhandovers across professional boundaries and receiving/making referrals asappropriate.

  • A senior clinician involved inthe delivery of a responsive, proactive, duty/same day service in the practice.
  • Communicate at all levelsacross organisations ensuring that an effective, duty/same-day service isdelivered and understood.

Person Specification
Qualifications
  • Holds a MSc advanced practice programme qualification accredited by the Centre for Advancing Practice or has completed the Centres ePortfolio (supported) Route or is enrolled on either programme/route.
  • Holds an Advanced digital badge or is on an approved programme for which a digital badge will be awarded.
  • Is registered with the Nursing and Midwifery Council.
  • Is able to practice as an advanced nurse practitioner.
  • If not a qualified non-medical prescriber, must be prepared to obtain the non-medical prescribing qualification.
  • Is an independent non-medical prescriber.
  • Teaching qualification.

  • Ability to effectively assess, plan, treat and refer patients appropriately and evaluate patient care confidently.
  • In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
  • An appreciation of the nature of general practice.
  • An appreciation of the NHS landscape including the relationships. between individual practices, PCNs and the commissioners.
  • Experience of prescribing, medication reviews and optimising treatments.
