Job Description Advanced Clinical Practitioner (ACP)
Job Summary
The Advanced Clinical Practitioner (ACP) is an experienced, registered healthcare professional who has been assessed as competent in advanced clinical practice through education at Masters level (or equivalent) and mapped against the national Multi-professional Framework for Advanced Clinical Practice.
The ACP works autonomously, with the authority to assess, diagnose, treat, prescribe, and manage patients presenting with complex needs, uncertainty, and risk, within the limits of their professional competence. They apply advanced clinical reasoning to support safe decision-making, while ensuring appropriate escalation or referral when necessary.
The ACP provides timely, patient-centred care and supports avoidance of unnecessary GP referral, contributing to improved access, continuity, and outcomes for patients. The ACP role is underpinned by the four pillars of advanced practice (clinical practice, leadership and management, education, and research). Protected time and structured job planning will ensure the ACP contributes to each pillar, supporting personal development, service transformation, and organisational objectives.
