The Advanced Practitioner will work as an autonomous practitioner using complex decision making skills. The post holders primary focus will be the provision of excellent patientcentred care, underpinned by the four pillars of clinical practice (competence and capability), leadership and management, education and research.
Care delivery will encompass the skills of telephone triage, remote assessment and face to face clinical assessment, examination, clinical diagnosis and the development and delivery of effective urgent treatment for patients attending with differentiated and undifferentiated individual circumstances and complex situations, undiagnosed primary/ urgent health care problems of full general practice patient needs including paediatrics, care of elderly and mental health.
The post holder will provide expert clinical advice, leadership and support to other clinicians ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis and advanced decision-making skills.
The post holder will be accountable for the direct delivery of autonomous service user care using advanced clinical knowledge that is evidence based. They will undertake independent comprehensive assessment and management of service users to incorporate pharmacological considerations, making complex clinical decisions regarding service user management and clinical outcomes. They will lead as a driver for change, to monitor and improve standards through supervision, evidence-based practice, clinical audit, research and education. This is a designated non- medical prescribing post.
