A healthcare provider in the UK is seeking an Advanced Clinical Practitioner for community in-patient units. The role involves providing high-quality, patient-centred care, including rehabilitation and recovery for patients. We are looking for a dynamic individual who is confident in their clinical practice and who can drive forward service transformation inline with the national agenda.
The post holder will work within the York and Selby Community in-patient units as part of Community and Care of the Elderly Services.
The aim and main function of the community in-patient unit bed base is to provide short term rehabilitation, reablement, recovery and end of life care to the patients.
The post holder will practice as an Advanced Clinical Practitioner within the Community Inpatient Units, they will join a Team of Advanced Clinical Practitioners and Trust Grade Doctor to provide patient‑centred, high‑quality care.
The role will encompass the skills of assessment, examination, diagnosis and treatment within this area pf practice. Supporting admission from the Hospital and the community and timely patient discharges from the in‑patient units.
