Advanced Clinical Practitioner (Clinical Pharmacist)

We are looking for a fixed-termAdvanced Clinical Practitioner to join our team. The rolewould suit a practice Clinical Pharmacist with experience wanting a fixed-term role. Locumsconsidered.

The role is to cover maternity leave, and is available for a fixed term of 9 months (or less if the staff member on maternity leave returns early). The post is funded via the Additional Roles Reimbursement Scheme (ARRS).

Main duties of the job

This role involves providing expert Clinical Pharmacist care to patients assessing patients, prescribing and reviewing medications, providing education and support to patients. As well as this, you will be required to contribute towards national clinical targets including IIF as well as prescribing guidelines.

Job responsibilities

Key Responsibilities:

  • Patient Assessment and Management - Conductingpatient assessments, including taking medical histories, performing physicalexaminations, requesting relevant investigations, developing and implementingmanagement plans, and making necessary referrals to other members of thePractice team. Manage undifferentiated undiagnosed conditions and identify redflags and underlying serious pathology and take appropriate action
  • Personalised Care use complex decision makingto inform the diagnosis, investigation, complete management of episodes of carewithin a broad scope of practice, and actively take a personalised careapproach and population centre care approach to enable shared decision makingwith the service user
  • Medication Management - Prescribing andreviewing medications for medicine optimisation of long-term conditions andstructured medication reviews.
  • Medication Safety - Identifying and mitigatingrisks associated with medication use for patients and the practice.
  • Patient Education and Support - Providinginformation and advice to patients on their medications, promoting adherence totreatment plans, and supporting self-management strategies.
  • Multidisciplinary Teamwork - Collaborating withother healthcare professionals, such as doctors, nurses, and other alliedhealth professionals, to optimise patient care and ensure a seamless approachto treatment. Attending weekly MDT meetings with our partners care home tocontribute and manage vulnerable patient health.
  • Service Development and Improvement -Participating in clinical audits and quality improvement initiatives includingIIF, QoF and Prescribing Incentive scheme. Working closely with the ICB toensure effective prescribing in line with local formulary.
  • Leadership and Education - Providing leadershipto the Pharmacy Technicians and wider clinical team. Maintain and develop owntraining and learning.
  • Encompass the four pillars of clinical practice;leadership and management, education and research, with demonstration of corecapabilities and area specific clinical competencies
