Job summary

An exciting opportunity is available to join Meir Primary Care Network as our Advanced Care Practitioner - Nurse or Paramedic.

The right candidate will be motivated, enthusiastic and experienced prescribing Practitioner who is able to work effectively within our clinical team to undertake patient assessment, diagnosis and clinical management.

Main duties of the job

The successful candidate should be an experienced practitioner who, acting within their professional boundaries and competencies, will provide care to the presenting patient, from initial clinical assessment, through diagnosis, treatment, and an evaluation of care. They will demonstrate safe and effective clinical decision making and critical thinking. Above all they will be a caring and compassionate clinician who puts the needs of their patient at the heart of everything they do.

About us

Meir PCN is a membership comprising of six practices - Adderley Green Surgery, Meir Medical Practice, Meir Park & Weston Coyney Medical Practice, Dr Raju & Partner, Trinity Medical Centre and Willow Bank Surgery - covering the Meir, Longton & Blythe Bridge areas of Stoke on Trent.

We currently have a PCN population of approximately 38500 patients and the PCN staff work in conjunction with the local general practices, local services and other organisations to support both local and national priorities.

We take immense pride in our PCN and how we are providing excellent support and care to our practice colleagues and patients.

Our goal is to improve health & care locally and to meet everyday health and care needs for patients by connecting primary care systems and using creative thinking to develop, improve and support great local services.

Job responsibilities

Recogniseand work within own competence and professional code of conduct as regulated bythe NMC / HCPC

Assess,diagnose, plan, implement and evaluate treatment/interventions and care forpatients presenting with an undifferentiated diagnosis

Assessand triage patients, including same day triage, and as appropriate providedefinitive treatment (including prescribing medications following policy,patient group directives, NICE (national) and local clinical guidelines andlocal care pathways) or make necessary referrals to other members of theprimary care team

Manageundifferentiated undiagnosed condition and identify red flags and underlyingserious pathology and take appropriate action

Usecomplex decision making to inform diagnosis, investigation, complex managementsof episodes of care within a broad scope of practice

Activelytake a personalised care approach and population centred care approach toenable shared decision making with the presenting person

Clinicallyexamine and assess patient needs from a physiological and psychologicalperspective, and plan clinical care accordingly

Assess,diagnosis, plan, implement and evaluate interventions/treatments for patientswith complex needs

Proactivelyidentify, diagnose, and manage treatment plans for patients at risk ofdeveloping a long-term condition (as appropriate)

Diagnoseand manage both acute and chronic conditions, integrating both drug- andnon-drug-basedtreatmentmethods into a management plan

Prescribeand review medication for therapeutic effectiveness, appropriate to patientneeds and in accordance with evidence-based practice and national and practiceprotocols, and within scope of practice

Work withpatients in order to support compliance with and adherence to prescribedtreatments

Provideinformation and advice on prescribed or over-the-counter medication onmedication regimens, side-effects, and interactions

Prioritisehealth problems and intervene appropriately to assist the patient in complex,urgent or emergency situations, including initiation of effective emergencycare

Supportpatients to adopt health promotion strategies that promote healthy lifestyles,and apply principles of self-care

Supportand manage health needs of women presenting for family planning, cervicalcytology or sexual health consultation

Implementand participate in vaccination and immunisation programmes for both adults andchildren

Communicateeffectively with patients and carers, recognising the need for alternativemethods of communication to overcome different levels of understanding,cultural background and preferred ways of communicating

Takeresponsibility for own learning and performance including participating inclinical supervision and acting as a positive role model

Activelypromote the PCN as a learning environment, encouraging everyone to learn fromeach other and from external good practice

Prioritiseown workload and ensure effective time-management strategies are embeddedwithin the culture of the team

Discuss,highlight, and work with the PCN to create opportunities to improve patientcare

Manageand lead on the delivery of specifically identified services or projects asagreed with the PCN

Applyinfection-control measures within the practice according to local and nationalguidelines

Act as arole model in the observance of equality and diversity good practice

Two clinical references will be requested as part of pre-employment checks
