As an ACP at Guttridge Medical Centre, you will be delivering high-quality, person-centred care across a blended role comprising both clinic-based consultations and home visits. You will work autonomously, drawing on your advanced clinical assessment and diagnostic skills, and collaborate closely with our experienced multi-professional team.
Your core responsibilities will include:
Assessing, diagnosing, planning, treating and reviewing patients with both acute and chronic conditions.
Carrying out home visits where required.
Managing clinical episodes independently including prescribing and referrals.
Taking a lead in unscheduled and emergency care.
Communicating effectively with patients, carers and clinical team.

You'll join a vibrant, multi-disciplinary team that includes 4 GPs,1 Clinical pharmacist, 1 Practice nurse, 1 GP Assistant, 1 Social prescriber,1 Medicines co-coordinator and practice staff.
