Advanced Clinical Practitioner (Band 8a) - Virtual Ward (Kirklees)
Pay: £35 PAYE (inclusive)
Contract: Temporary
Hours: 08:00-16:00 or 09:00-17:00
Location: Kirklees

Service Care Solutions are recruiting a highly skilled Advanced Clinical Practitioner (ACP) to join the Virtual Ward service in Kirklees. This role focuses on delivering exceptional care to patients with complex needs, including frailty and heart failure, while supporting safe discharge and reducing avoidable hospital admissions.

Key Responsibilities Clinical Delivery
  • Provide comprehensive assessment, diagnosis, treatment, and evaluation for Virtual Ward patients.
  • Lead on heart failure management within the service.
  • Independently prescribe and review medication (including IV prescribing).
  • Identify early deterioration, acute illness, and frailty needs using advanced clinical reasoning.
  • Support end‑of‑life care planning and guide patients, families, and care homes.
  • Manage acute and chronic conditions using evidence‑based interventions.
Care Coordination
  • Work collaboratively with acute trusts, community teams, specialist nurses, therapy teams, primary care, and social care.
  • Facilitate safe discharge pathways and integrated care planning.
  • Participate in MDTs and communicate complex clinical information effectively.
Leadership & Professional Practice
  • Provide senior clinical advice and mentorship to nursing and HCA colleagues.
  • Contribute to clinical governance, audit, research, and service improvement.
  • Maintain accurate clinical documentation and uphold safeguarding responsibilities.
Requirements Essential
  • MSc in Advanced Clinical Practice (or equivalent).
  • Registered Independent Prescriber (V300).
  • Advanced skills in frailty and heart failure management.
  • Competency in physical assessment, diagnostic reasoning, phlebotomy, and autonomous decision‑making.
  • Experience in community or acute settings.
  • Full UK driving licence and ability to travel across Kirklees.
Desirable
  • Experience in elderly care, CHD, diabetes, respiratory, or end‑of‑life care.
  • Clinical supervision, audit, and teaching experience.
  • IT skills including SystmOne.
